Project Title: Enhancing Adolescent Wellness: Comprehensive Clinical and Mental Health Support at the Dr. Robert Appleby School-Based Health Centers (SBHC)
Project Description:

The Human Services Council (HSC) requests $10,000 to support the Dr. Robert Appleby School-Based Health Centers (SBHC), which provide critical medical and mental health services directly within Norwalk’s middle and high schools. By offering "one-stop" healthcare, we eliminate traditional barriers to access—such as lack of transportation, insurance gaps, or parental work schedules—ensuring that students receive high-quality care without missing significant instructional time.

The SBHCs are staffed by multidisciplinary teams, including Mid-level Practitioners and Licensed Clinical Social Workers. Funding from the Hermanowski Foundation will specifically support the delivery of integrated care:

Mental Health Services: Individual and group counseling to address rising rates of anxiety, depression, and trauma among adolescents.

Clinical Medical Care: Physical exams, immunizations, and acute care for illnesses or injuries.

Health Education: Proactive wellness coaching to encourage lifelong healthy habits.

Our centers serve as a vital safety net, particularly for students who are underinsured or uninsured. By addressing health issues early and in a familiar environment, we reduce ER visits and improve school attendance. This grant will help cover the gap between insurance reimbursements and the actual cost of providing intensive, trauma-informed care to Norwalk’s most vulnerable youth, ensuring that every student has the physical and emotional foundation needed to succeed academically.

Total Project Budget: 60,000

Other Funding
Sources For The Project (Committed & Potential): Connecticut Department of Public Health (DPH) grants and billing reimbursements from third-party insurance. Potential: Neighborhood Assistance Act (NAA) tax credit contributions and private individual donations.

Project Duration: 12 months
Geographical Area Served: Norwalk, CT
Age Group To Be Served: 10-20

Project Title: Afterschool Performing Arts Program for Underserved Youth in Dallas
Project Description:

Texas Supremacy of Music & Arts Conservatory (TSMAC) seeks support to expand its Afterschool Performing Arts Program serving underserved youth in Dallas, Texas. The program provides free or low-cost training in step, dance, drill, music, and leadership development for middle and high school students from low-income communities.

Through structured afterschool programming, students receive professional instruction, mentorship, and opportunities to perform in community events and competitions. The program promotes discipline, teamwork, creativity, and self-confidence while offering a safe and supportive environment during afterschool hours.

Many youth in Southern Dallas face limited access to arts education due to funding cuts in schools and economic barriers. TSMAC addresses this gap by providing culturally relevant performing arts instruction that keeps students engaged, motivated, and connected to positive peer networks.

Funding will support instructor stipends, rehearsal space, uniforms, performance equipment, and program supplies. The program is expected to serve approximately 50–75 students annually through weekly practices, workshops, and performances.

By investing in performing arts education, this program helps youth build valuable life skills, improve academic engagement, and develop pathways toward leadership and creative expression within their communities.

Project Title: Growing Bright Futures: Volunteer Advocacy for Abused and Neglected Children
Project Description:

For 30 years, Chesterfield CASA has served children who have experienced abuse and neglect in Chesterfield County, Virginia. Our mission is to advocate for the best interests of children involved in the juvenile court system so that every child has the opportunity to grow up in a safe, stable, and permanent home.

Our core program focuses on recruiting, training, and supervising community volunteers to serve as Court-Appointed Special Advocates for children in foster care due to abuse or neglect. Once trained and sworn in by the court, each volunteer advocate is assigned to a child or sibling group, at no cost to the family, and remains involved throughout the duration of the case, providing a consistent, caring presence during what is often a confusing and uncertain time.

Under the supervision and support of professional staff, advocates build trusting relationships with the children they serve through regular visits. They also gather information from parents, caregivers, social workers, service providers, and others involved in the child’s life to gain a full understanding of the child’s circumstances. Advocates synthesize this information into objective reports and recommendations submitted to the court, helping judges make informed decisions about safety, services, and permanent placement. In addition, advocates monitor court orders and case progress to ensure services are provided, needs are addressed, and the child’s well-being remains the priority throughout the life of the case.

In 2025, Chesterfield CASA served 152 children across 82 families. Our advocates provide reliable information and monitor progress throughout each case, which helps reduce time spent in the child welfare and juvenile court systems and promotes decisions that lead to safe, stable, and permanent homes. This individualized advocacy ensures that children who might otherwise feel invisible are seen, heard, and given the opportunities needed to thrive.

Project Title: CIS Rowan
Project Description:

Communities In Schools of Rowan (CIS Rowan) provides comprehensive, school‑based, integrated student supports designed to help students overcome non‑academic barriers and stay on track to graduate. Through a proven model that embeds trained Site Coordinators inside partner schools, CIS Rowan delivers a continuum of services that includes school‑wide prevention supports, targeted group interventions, and individualized case management for students facing academic, social, emotional, and basic‑needs challenges.

CIS Rowan currently serves 12 partner schools across Rowan County and Kannapolis City Schools. Students receive coordinated supports such as mentoring, tutoring, attendance interventions, behavioral skill‑building, and access to essential resources,s including food, clothing, hygiene items, and transportation assistance. These services help stabilize immediate challenges while strengthening protective factors that improve attendance, behavior, coursework performance, and long‑term engagement in school.

In addition to academic and basic‑needs support, CIS Rowan provides robust college-and-career readiness programming. Through initiatives such as My Future Me, college tours, workforce exposure opportunities, and leadership development workshops, students gain the knowledge, confidence, and real‑world experiences needed to pursue postsecondary education, technical training, or employment. These programs help students understand how their education connects to future goals and community impact.

CIS Rowan is also preparing for expansion into two additional schools during the 2026–2027 school year in response to rising student needs and requests from district leadership. This growth will allow the organization to reach more youth experiencing chronic absenteeism, academic struggles, and family instability.

Proposal Information

Today’s Date: 3/5/2026
Requested Amount: $3000

Project Title: Workforce Development, Food Service, Life Skills, and GED Training Program
Project Description:

Life’s Kitchen is a nonprofit social enterprise providing Opportunity Youth (formally at-risk youth) 16-24 years of age with wrap around life and work skills in Boise Idaho. Opportunity Youth are young adults who are not in school and not working.
Failure to complete high school affects a young adult’s ability to become financially independent. Job opportunities are scarce for young adults who have inadequate job skills, lack experience, or do not have a high school diploma. Even if these at-risk young adults can find employment, they often lack the basic life skills that would allow them to navigate the workplace, advance in their career, manage their money, and become financially stable.

Through our Workforce Development, Food Service, Life Skills, and GED Training Program, trainees receive valuable skills and resources they need to become independent, financially stable, and successful.

Funding from The Charles and Joan Hermanowski Family Foundation will be used to support training for up to 65 young adults and ongoing support for more than 85 alumni over the next year. The funds will help cover educational, computer, and transportation expenses; GED and ServSafe tutoring and testing; kitchen supplies; trainee meals and support; training support staff; and core program support including staff salaries, utilities, and other associated hard costs of running the organization.
