Project Description:

Jacksonville Speech & Hearing Center (JSHC), a small community-based nonprofit founded in 1949, requests $10,000 to support of Fund-A-Future, our subsidized speech-language therapy program. As the only regionally accredited provider of speech-language pathology and audiology services for all ages, and the only Jacksonville healthcare organization with a Certified Autism Center designation, JSHC directly addresses critical health and developmental needs for children.

Fund-A-Future provides free comprehensive evaluations and weekly therapy to uninsured, underinsured, and low-income children ages 3-12 facing communication disorders. Delivered by certified Speech-Language Pathologists, sessions incorporate evidence-based practices and caregiver training to extend progress at home.

Duval County faces severe challenges: estimates indicate 10,240-15,360 children under 5 require speech-language intervention. In 2024, kindergarten readiness hovered at 45.8%. In 2025, JSHC served 2,099 speech-language patients (98+% under 18), delivering 14,750 therapy sessions (43% grant-funded).

This $10,000 will fund 200 additional 30-minute speech therapy sessions ($50 each), allowing us to increase our total Fund-A-Future capacity to 1,000 sessions annually. This expansion ensures that at least 50 low-income children receive a consistent, high-impact dosage of weekly therapy and caregiver training. By providing this intensive support now, we move the needle on Jacksonville’s 43.8% kindgergarten readiness rate and provide a projected $100,000 in long-term savings for our local school system. We aren’t just funding sessions; we are ensuring children enter the classroom ready to succeed.

Outcomes include improved communication skills (measured via standardized assessments), enhanced school readiness, stronger caregiver confidence, and transformed life trajectories for vulnerable children.

When a child gains their voice through targeted support, we change not just their childhood but their future potential.

Make Promises Happen, a Twin Cedars Camp, provides quality, engaging camping and outdoor recreation programs for children and adults with physical and intellectual disabilities. We believe that the experiences individuals have in the outdoors, learning new skills, building new relationships, and getting outside of their comfort zones, are truly transformational, and that ALL people, regardless of ability, background, or identity. In 2025, this program served 693 participants and provided more than 48,000 hours of respite care to parents and full-time caregivers of campers with disabilities.

The cost for a camper and their one-to-one volunteer to attend a week of camp is $700. Our organization has never turned a camper away due to a lack of funds. A gift of 8,400 from the Hermanowski Foundation would provide full scholarships for 12 campers and their one-to-one volunteer caregiver for a week of summer camp.

The Forever15 Fund, led by Baltimore’s Promise, provides rapid-response emergency support to young people facing immediate financial crises that threaten their safety, stability, and ability to stay on a path toward economic mobility. Designed in partnership with youth and informed by lived experience, the Fund fills critical gaps that traditional systems and funding streams cannot address quickly enough.

Young people in Baltimore frequently encounter urgent barriers, housing instability, food insecurity, transportation breakdowns, unmet mental health needs, and unexpected costs such as utilities, childcare, or essential documents. Even relatively small financial disruptions can derail a young person’s ability to remain engaged in school, workforce training, or employment. At the same time, access to safety-net supports continues to decline, particularly in low-income and immigrant communities, making flexible, rapid-response resources even more essential.

The Forever15 Fund is intentionally structured to respond within days, not weeks, ensuring that young people receive timely, flexible support when it matters most. Through this Fund, Baltimore’s Promise provides direct assistance for needs such as emergency housing support, utility payments, transportation access, work-related expenses, and other critical necessities. Requests are reviewed through a streamlined, equity-centered process that prioritizes dignity, trust, and youth voice.

A $10,000 investment will directly support dozens of youth/young adults with emergency assistance, depending on need, helping them remain connected to education, career pathways, and essential supports. Beyond immediate relief, the Fund serves as a stabilizing force, preventing crises from escalating and enabling young people to continue progressing toward long-term goals.

Project Title: Rapid Response for new Families
Project Description:

Congenital Central Hypoventilation Syndrome (CCHS) is a rare, life-threatening genetic disorder affecting the autonomic nervous system, causing children to lose automatic control of breathing, particularly during sleep. With an estimated incidence of 1 in 200,000 births and fewer than 1,500 known cases worldwide, CCHS is so rare that most physicians will never encounter it in their careers. Yet the consequences of mismanagement are severe: respiratory failure, cardiac arrest, and death.
The most urgent barrier facing CCHS families today is not the absence of treatment options. It is the inability to find a physician who has ever seen another patient. Families spend months or years navigating a fragmented medical system, often relying on local pulmonologists or neurologists with no CCHS experience, while a small cluster of knowledgeable specialists exists at a handful of academic medical centers with no roadmap to reach them. Families in states like Florida, where CCHS cases are documented but expertise is sparse, face this crisis acutely.
CCHS Network requests $10,000 to design, build, and publicly launch the first-ever CCHS Specialist Network Map: an interactive, searchable online tool that allows families of children diagnosed with CCHS to identify physicians with documented CCHS experience by state, specialty, and telehealth availability. The project will include a structured outreach survey to the national CCHS clinical community, a web-based mapping tool embedded in the CCHS Network patient portal, and a plain-language family navigation guide to help newly diagnosed families use the resource and initiate specialist consultations.
This tool will be built once and serve families in perpetuity. For a child whose life depends on proper ventilator management and expert longitudinal care, knowing where to find a physician who understands their condition is not a convenience. It is a lifeline.

Project Title: The Agri -STEM outdoor Learning
Project Description:

Smith Family Ranch Foundation provides a safe and engaging environment where youth and families can connect with agriculture, nature, and hands-on learning experiences. In many underserved communities, young people have limited exposure to food systems, farming, and outdoor education, which restricts both life skills development and awareness of agricultural career pathways.

This project will expand our youth-focused agricultural education program in Stockbridge, Georgia. Through structured, hands-on activities, participants will learn animal care, gardening, food production, and basic agribusiness concepts. The program also promotes leadership, responsibility, teamwork, and healthy living.

Funding will support program materials, farm supplies, educational tools, and operational costs necessary to increase participation and improve program delivery. It will also help reduce barriers to access for underserved youth and families.

Over the project period, we aim to serve at least 100 youth and families, improving their understanding of sustainable agriculture and strengthening their connection to healthy food systems.
