Project Title: Grassroots Health: College Athletes Decreasing Mental Health Stigma in Schools
Project Description:

Hello Hermanowski Foundation,

I hope all is well with you. I am reaching out because I came across the foundation’s work, and was hoping we might be aligned – although we are not based in Florida.

I founded and lead Grassroots Health, an innovative, youth-led organization that mobilizes college athletes to serve as volunteer health educators in schools. We currently operate in Washington, DC, and Philadelphia, reaching approximately 1,200 low-income and at-risk students per year. Our evaluations have shown our programs to be effective in increasing students’ mental health literacy, decreasing mental health stigma, and improving students’ access to mental health services.

With mental health challenges for youth significantly increasing over the past 3 years, we have seen a higher demand for our programs than ever before, and we often have to tell schools that we do not have the capacity to bring our programming to them due to limited funding.

I was hoping we might be able to apply for a $5,000 or $10,000 grant from the foundation to support our expansion to more schools on our waiting list.

Please let me know if you might be open to a full application. I am also happy to talk more about our program model, evaluations, budget, etc.

Thank you so much for considering our request!

All my best,

Tyler

Proposal Information

Today’s Date: 3/27/23
Requested Amount: $10,000

Project Title: TBRS Summit Patient and Family Travel Scholarships
Project Description:

Tatton Brown Rahman Syndrome (TBRS) is a rare, neurodevelopmental disorder that affects almost every system in the body. Affected individuals struggle with the isolation of having a relatively unheard of rare disease, while facing a diagnosis that makes them 250 times more likely to develop cancer. 100% of people with TBRS have intellectual disability. Other health impacts include seizures, cardiac issues, orthopedic problems, and blood-related issues. As a result, education and support are crucial for these individuals.

We would like to offer Travel Scholarships to patients and families with TBRS to allow them to attend the 2023 TBRS Summit. These scholarships will be offered to those with the need for financial support. The Summit offers a unique opportunity for patients to interface with researchers, and we want to ensure there is no financial impediment to attendance. Having patients in attendance to speak with researchers and clinicians will not only educate them on resources and information from a scientific perspective, but also allows patients to educate others on the personal perspectives of life with TBRS. Additionally, we have planned family-friendly educational events, including presentations from scientists, long Q&A panels to enable patients to ask questions in a comfortable environment, family and researcher poster sessions, and assistance in accessing the resources TBRS Community provides to patients.

The TBRS Summit will be held in San Antonio, TX, at Morgan’s Wonderland. Morgan’s Wonderland is committed to accessibility for individuals with disabilities, which provides our patients with a comfortable and accommodating environment for the conference. We will offer virtual access for patients that are not able to attend, but it is crucial to have patients in attendance for their own educational, emotional, and personal benefit. Support from the Hermanowski Family Foundation would go a long way to supporting patients in an immediate, tangible way.

Total Project Budget: $75,000

Other Funding
Sources For The Project (Committed & Potential): Committed: Morgan’s Wonderland $20,000, CZI Rare As One Grant $15,000; Potential: WITH Foundation Open Grant $30,000, Horizon Therapeutics #RAREis Global Advocate Grant $5,000

Project Duration: Thursday 10/12 – Sunday 10/15 (4-5 days with travel)
Geographical Area Served: United States; Families are located throughout the globe, with more than 50% of our population being from the United States. Patients outside of the United States will be supported by other grants and TBRS Community funding.
Age Group To Be Served: Patients between the ages of 0 and 21 will be served by these travel grants. Due to the relatively new discovery of TBRS, most patients are children and young adults (80% of patients are ages 21 or under). Individuals over 21 will be served by other grants and TBRS Community funding.

Project Title: Mentor Program
Project Description:

Blueprint 58 is a community-based development program in the Adair Park and Pittsburgh neighborhoods of Atlanta. Blueprint 58 was birthed from the belief that lasting relationships are the only context for true community and heart transformation. Our mission is to strengthen our community by empowering youth, equipping young adults and families, and engaging neighbors.

One way we strive to intentionally build these relationships and empower youth in our community is through our one on one mentoring program. Here at Blueprint 58, we take on a different definition of mentoring; we believe that both our students and mentors have much to learn from each other. Our mentoring program is based on the idea that our mentors may have much knowledge to bestow to their mentees, but they have equally as much to learn from their mentees. Mentoring for Blueprint 58 is about sparking mutually transformational relationships in the hopes that our mentors and students learn and grow together. Blueprint 58’s mentoring program matches 4th grade students in the Pittsburgh and surrounding neighborhoods of Atlanta with local mentors. Our program specifically focuses on building relationships within our community.

Blueprint 58 mentors are required to see their students at least twice a month and for at least 4 hours a month. Mentors are also expected to communicate with their student weekly. Our mentoring program is based upon the hope that mentors will build organic relationships with their students and that both students and mentors become part of each other’s families. However, we know that starting these relationships and building them up can be challenging, so we are committed to supporting and encouraging our mentors and students throughout their mentoring relationships. Blueprint 58 provides ongoing support to mentor pairs through fellowship events, mentor appreciation events, ongoing training for mentors, and parent engagement events.

Blueprint 58 is committed to our mentored students through high school, regardless of whether a mentor can fulfill their commitment or not. Once a child is paired with a mentor, we will continue to match them with a mentor throughout their middle and high school career.

Project Title: IMPACT Your Acceptance (Recruitment)
Project Description:

The IMPACT Your Acceptance (IYA) program helps youth and their families become knowledgeable about the college application process and provides them with a suite of resources to ensure they achieve their goal of college acceptance and completion of a degree. IYA instills confidence in the minority, working-class students it serves, helping them become fully college-ready as they progress through grades 10-12. We continue to serve participants through their college process, to the extent that they desire.

IYA increases participants’ odds of being accepted to their target university programs and helps them to secure enough funding to complete their studies. The goal is to help youth conquer the battle of being accepted into college and to win the war of completing higher education. The IYA program does this by helping scholars secure funds for tuition and living expenses and providing mentorship and support networks that ensure they stay on track and follow through.

In preparation for the college application season, IYA students undergo several weeks of virtual class sessions and hours of one-on-one sessions with writing coaches to perfect their essays and personal statements. Parents and students both receive guidance around completing FAFSA and choosing scholarships. Support from Charles And Joan Hermanowski Family Foundation would help us scale IYA to serve a cohort of 50 students this year, which would be our largest cohort yet.

Project Description:

The Rose Arts Festival “RAF” was originally launched in 1965 and became a huge family success. By the time it ended in 1998, it was a two-week family festival with many successful events.

In 2017, a group with a passion to see it reinstated brought it back to Norwich. For three years, it was a successful and growing event, until the pandemic hit. During the interim, RAF raised funds to create backpacks for the children who were food insecure. RAF was back in 2022 and was a resounding success.

The RAF committee is committed to keeping the Rose Arts Festival activities on Chelsea Parade free, so no one is denied access due to their ability to pay. RAF believes in diversity, no matter what role or function a person performs. We are committed to achieving this through diversity among our partners, staff, and partnering with companies and people who exemplify these values. We oppose discrimination, inequality, and injustice and are proud to represent equality for all. RAF is committed to supporting the local community by only purchasing products and working with vendors from the local community whenever possible.

Project Title: KIDS FIRST! Film Critics
Project Description:

We are seeking funding to support educating tweens and teens to become active members of the KIDS FIRST! Film Critics program. This project will provide training and on-going support for 4 tween and teens from low- and moderate-income families, 2 from Florida and 2 from the Washington DC area. These young people will be trained as journalists and entertainment reporters and coached with every assignment they undertake for a period of one year, beginning September 1, 2023.

Established in 2011, KIDS FIRST! Film Critics are nationally recognized as an authentic source of media reviews from young people, ages 9 to 20. These youth are trained by our professional educators and coached by both writing and video instructors starting with an initial 6 day boot camp, and continuing with every assignment. Beginning with something that they all love – watching films or online media – the kids learn to “watch, analyze, and review” what they watch and learn the skills needed to express their opinions with honesty, integrity, and insight. Recent reports from six graduates of the KIDS FIRST! program with young adults who are now actively working in the entertainment and journalism industry have shown the positive effects that their experience made on their success in college and now, as working professionals.

Their training takes place in a safe and supportive environment of peers and role models where students learn to see themselves as entertainment journalists. They learn the concepts of responsible journalism and open source reporting that forms the basis of all their future reporting. They work in teams to complete projects that include real media releases.
