Project Description:

In Jacob’s Shoes (IJS) has been providing basic necessities (shoes, backpacks, school supplies, & athletic gear) to local children since 2009. Our provision of services specifically targets children who are homeless/at-risk of becoming homeless, those living in poverty, on reduced/free lunch at area schools, living in homeless shelters, and/or living within the foster care system. In Jacob’s Shoes services are wholly inclusive and we do not discriminate based on race, color, religion, abilities, national and ethnic origin, or gender or for any reason. Necessities are provided to the described population by requests from schools, shelters, charities, and various other social service organizations and agencies as well as through public back-to-school events and campaigns. Since inception, we have proudly provided 236,000 pairs of shoes, 65,000 socks, and 12,000 backpacks (with school supplies) to local children living in poverty, suffering from displacement, and who are in the greatest need.
There are children in our area who have issues with absences and show up late to school because they do not have adequate shoes or, in some cases, must share shoes. There are many more wearing shoes that don’t fit or have missing/detached soles. They are cruelly teased because their shoes are old, dirty & torn.
In Jacob’s Shoes receives in-kind donations of gently worn shoes which our volunteers collect, clean, & restore to pristine condition & then we supplement these with purchased shoes. Shoes & backpacks are distributed via requests made by schools, shelters, charities & social service agencies working directly with our target population. Requested items are ready for pick-up by the school/agency within one to five days. Representatives from the schools & agencies deliver the shoes to the children for whom the request was made. The process is fast, efficient & anonymous, saving the child from unnecessary embarrassment.

Project Title: Brain & Body Integration Program for Children
Project Description:

For fifty-two years, the Rehabilitation Center for Neurological Development has been offering Brain & Body Integration Program for Children. The program is designed to assist children diagnosed with birth trauma, traumatic brain injury, autism, ADHD, learning disabilities, Down syndrome or other brain related condition that resulted in a developmental delay. Services offered by the rehabilitation program are individually designed to stimulate each client’s brain and “rewire” the damaged or underdeveloped areas. Rehabilitative services may include neurodevelopmental exercises, cognitive/language development and tutoring, visual training and sensory training. The program approach focuses on treating the problem not the symptom.

The program enrolls a minimum of thirty children in the program annually. Clients attend the program 2 days per week for the 8-week period. Services are provided on an individual basis and may include but are not limited to the following: neuroeducational class, neurodevelopmental exercises, vestibular exercises, perceptual/language/cognitive training, educational kinesiology, and aquatic therapy.

Each child completes a pre and post evaluation to monitor progress. Based on the evaluation, an individual service plan is developed that specifically addresses that child’s unique needs. Services offered in the 8-week program may include but are not limited to the following: Neurodevelopmental Exercises, Vestibular Exercises, Perceptual/Language/Cognitive Training, Educational Kinesiology, and Aquatic Therapy. These services are provided on an individual basis. Services are provided regardless of ability to pay.

By program completion, each child shall achieve at least one of the following measurable objectives: improved upper body strength, improved lower body strength, improved balance, improved coordination, improved mobility, improved cognitive processing, increased language skills, improved visual /auditory processing or improved neurological organization.

This grant award shall extend the program through 2025. The program shall target those families that have limited financial resources and may otherwise not be able to take advantage of the services offered through the Center.

Boca Raton Achievement Center (BRAC) is a private, non-profit, specialized school for students with varying abilities and challenges. Some of these challenges are academic, and others are sensory, social and/or emotional. Although many of our students are on the autism spectrum, we are not a school for autism. We are a school for any student with learning disabilities, unable to succeed in a typical school setting.
BRAC provides a tailored education for our middle and high school students, ages 11 – 22. BRAC is not just a school, it’s a personalized learning environment. We understand that each student has unique needs and strengths. Our curriculum and instructional approach are meticulously tailored to cater to these individual requirements, ensuring that every student receives the support they need to thrive.
At BRAC we go beyond academics. Our programs foster holistic development by offering comprehensive instruction in academics, life skills, health and fitness, independent living, and vocational training. Our goal is to equip students with the skills and knowledge necessary to lead fulfilling lives beyond the classroom.
As a private school, tuition can be a challenge for many families. Although state funding is available for some our students, the families are still responsible for the difference, which can be anywhere from 7,000 to 18,000 per year. Many families struggle with this, and unfortunately cannot enroll their child due to financial constraints. Our goal is to create a scholarship fund that will bridge the gap for families unable to afford the tuition. We want our school to be for those who need it, not just those who can afford it.

Project Title: Children’s Cancer Navigation
Project Description:

Palm Beach County has one of the highest percentages of pediatric cancer in Florida. This tragic diagnosis, and the vast amount of overwhelming information, courses of treatment, and necessary medical procedures, often leaves the parents/caregivers of these children overwhelmed. Delays in finding the best medical resources for their child’s treatment can have a significant impact on their child’s survival and quality of life.

Our Patient Navigator Program (PNP) assists these parents in finding the best options quickly, leading to an enhanced well-being of their child throughout the treatment process. A trained individual, the Patient Navigator (PN) helps identify & resolve barriers to care and enables parents to adhere to care recommendations for their child, thus improving their long-term health. The PN helps qualify these families for financial assistance, as well as guides them through the process of providing clients with emergency financial assistance.

Additionally, while generally helpful for everyone, navigation plays a vital role in allowing low-income, marginalized families increased access & utilization of cancer care.

Our Patient Navigator also provides the following:

• Assist parents in understanding charges on their bills.
• Provide information on government & private programs, including CAHH, that help pay costs.
• Provide information on procedures/treatments. Provide education to better understand what is happening with healthcare so they can make the best decisions.
• Translate materials so non-English speaking patients can understand treatment options.
• Helps qualify families for emergency financial assistance.

Through our comprehensive continuum of services, including in-home/onsite patient care navigation & emotional/practical support, our Patient Navigator helps each client achieve the highest outcomes for their children stricken with cancer.

We are writing to express our interest in partnering with your foundation, whose focus on all aspects of a child’s life up to age 21 aligns perfectly with our mission at New Life UBC Church: to nurture and empower future generations through education and the arts.

Many young people in our community, particularly middle and high school students and post-secondary youth, lack access to quality STEM and arts programs, which can limit their future opportunities. This is where our STEAM Engine OST (Out of School Time) Program comes in.

Our innovative program will provide these young people with the skills, knowledge, and experiences they need to succeed in college and careers. Through hands-on activities in science, technology, engineering, arts & math, and partnerships with community organizations, high-education institutions, and trade schools, we will spark their interest in STEM fields, cultivate creativity and analytical skills, and offer valuable internships and workshops.
