Project Title: Children’s Cancer Navigation
Project Description:
Palm Beach County has one of the highest percentages of pediatric cancer in Florida. This tragic diagnosis, and the vast amount of overwhelming information, courses of treatment, and necessary medical procedures, often leaves the parents/caregivers of these children overwhelmed. Delays in finding the best medical resources for their child’s treatment can have a significant impact on their child’s survival and quality of life.
Our Patient Navigator Program (PNP) assists these parents in finding the best options quickly, leading to an enhanced well-being of their child throughout the treatment process. A trained individual, the Patient Navigator (PN) helps identify & resolve barriers to care and enables parents to adhere to care recommendations for their child, thus improving their long-term health. The PN helps qualify these families for financial assistance, as well as guides them through the process of providing clients with emergency financial assistance.
Additionally, while generally helpful for everyone, navigation plays a vital role in allowing low-income, marginalized families increased access & utilization of cancer care.
Our Patient Navigator also provides the following:
• Assist parents in understanding charges on their bills.
• Provide information on government & private programs, including CAHH, that help pay costs.
• Provide information on procedures/treatments. Provide education to better understand what is happening with healthcare so they can make the best decisions.
• Translate materials so non-English speaking patients can understand treatment options.
• Helps qualify families for emergency financial assistance.
Through our comprehensive continuum of services, including in-home/onsite patient care navigation & emotional/practical support, our Patient Navigator helps each client achieve the highest outcomes for their children stricken with cancer.
