Project Title: Perry County Memorial Hospital Youth Path
Project Description:
The Perry County Memorial Hospital (PCMH) Youth Path is a comprehensive program designed to ensure clinical continuity for families in Perry County, from the first ultrasound through high school graduation. The program’s initial “Roots” Phase is a prenatal-to-postnatal support bridge, helping families maintain PCMH as their medical home, even as mother delivers at hospitals near by who provide labor and delivery services.
Phase 1: The Bridge (Prenatal to Age 5) begins at PCMH-affiliated clinics, where expectant mothers receive a “Delivery Travel Kit.” This kit, tailored for families traveling out-of-county to deliver, includes a $25 gas card, a “My First Journey” baby book, and checklists for transferring records back to PCMH pediatrics. Once home, families are welcomed with a scheduled one-week wellness visit at a local PCMH clinic, where they receive a “New Parent Survival Guide” highlighting local resources and pediatric milestones.
Phase 2: Growth (Ages 6-12) focuses on nutrition and physical activity. Children receive a “PCMH Passport to Health,” a logbook distributed through Perry County schools. Kids earn stamps for attending well-child checkups, participating in local sports, and tracking healthy habits. Achieving “Gold Status” earns them a PCMH-branded prize, reinforcing healthy behaviors.
Phase 3: Horizon (Ages 13-18) addresses behavioral health and resilience through quarterly peer workshops on topics such as substance prevention, digital wellness, and sports medicine. Each year, the “Future of PCMH” scholarship awards $500 to a graduating senior pursuing a health-related degree.
Digital support includes a QR-code resource hub and SMS reminders, ensuring families stay connected to PCMH at every stage, nurturing lifelong health and loyalty within the community.
