Project Title: Patient Navigation for Pediatric Cancer Patients
Project Description:

This program helps pediatric cancer patients, & their families, navigate & access local resources that increase quality of life, survivability, & optimize treatment outcomes. Headed by a Patient Navigator (PN), a trained individual who helps overcome barriers to care, enabling patients to adhere to care recommendations & improving their long-term results. The PN meets with families & conducts an initial interview/assessment to determine their unique needs.

The PN helps the child understand services, provides directions, & refers them to medical services. The PN helps them utilize comprehensive services which can include arranging transportation to doctor’s appointments, obtaining wellness items, ensuring timely access to treatment & procedures, finding & providing translative services (the assistant PN speaks both Spanish/English), & locating financial resources for unpaid medical costs.

Program goals include:
• Reduce the number of families with children at risk of homelessness due to a cancer diagnosis.
• Improve medical outcomes by providing their families with personal assistance in navigating their child’s cancer care, & by providing linkages to other resources such as support groups, prescription assistance, local/national hospitals, & organizations.
• Decrease reliance on Emergency Department care by increasing the family’s ability to meet the child’s care plan & providing resources that improve wellness.
• Ease the discomforts associated with cancer, & lower psychosocial distress scores for children diagnosed with cancer by providing cancer specific health & wellness items their families could otherwise not afford, including head wraps/scarves, wigs, skin creams, or chemo beanies.
