Project Title: Hope & Opportunity – Street Outreach Program
Project Description:

Interface Children & Family Services (Interface) seeks continued funding to enhance its Street Outreach Program (SOP), which supports homeless youth across Ventura County, California. The SOP offers a comprehensive continuum of care that addresses the immediate needs of homeless youth while empowering them to create lasting, positive changes in their lives. Ventura County’s high living costs and limited affordable housing contribute to a growing population of youth who are homeless, system-involved, or at risk due to trauma, exploitation, or human trafficking.

The program manages the Youth Hotline, conducts regular street outreach routes, and provides a range of services that include assessment, harm reduction, crisis stabilization, case management, and aftercare, all designed to help youth move toward stable housing. Youth are connected to family reunification, emergency shelters, transitional housing, or specialized shelters for human trafficking and domestic violence survivors, depending on their needs.

The SOP utilizes a strength-based and trauma-informed protocol aimed at enhancing the social and emotional wellness of homeless youth. Staff are trained in evidence-based practices like Trauma-Informed Care, Motivational Interviewing, and Positive Youth Development. Youth are actively involved in their care plans, and both Spanish- and English-speaking youth receive support from experienced outreach specialists to identify their needs and goals. Recognizing that youth face varying levels of need, staff design personalized action plans that address both short-term and long-term goals. Some youth require brief support to resolve family issues, while others need intensive, long-term services to address challenges like mental health issues, substance use, past trauma, and life skills deficits. Each client’s plan is tailored to provide the most effective resources and support to ensure positive outcomes.

Interface’s 51 years of expertise and its established relationships with community organizations, law enforcement, and county agencies will help expand the program’s impact and better serve homeless youth in the county.
