Project Title: Child Health Education and Supports
Project Description:
Child Health Partnership (Child Health) provides at-home support to children and parents to promote the health and well-being of families in our community. Our three primary program goals are:
1. promoting child and family health through health assessments, education and improved access to care;
2. encouraging positive parent-child interactions and child development through education and hands-on activities that build parent confidence and abilities, leading to school readiness;
3. supporting family self-sufficiency through employment and connections to community resources.
Child Health staff establish long-term, trusted relationships with families with babies and children age 0-6. Our teams of community health nurses and family support specialists provide comprehensive family supports that include health education, screenings and improved access to health, mental health and dental care. Child Health nurses teach the value of prenatal care and self-care during pregnancy, to promote healthy full-term babies. Staff encourage parent knowledge of how to use a medical home for their child, and how to navigate health and social service systems. We improve access to services, helping parents to problem-solve and advocate for their families.
Child Health staff meet with families in their homes at least monthly, to build knowledge, skills and resilience. We work with families in both rural areas and vulnerable urban communities, including many refugees and immigrants.
After one year of services in the program (2024 data), families show:
• 97% of children have an established medical home
• 92% of children are current on developmental screenings
• 89% of children are current on well child visits
• an 80% decrease in children who needed medical care and didn’t receive it.
We anticipate serving 350 children from 220 families in the coming year.
