Project Title: Transforming Children’s Lives through Care Coordination Services
Project Description:

Boys Town South Florida is seeking a $10,000 grant to support Care Coordination Services in Palm Beach County. Our Care Coordination Services transform lives, providing a lifeline for children who are suffering from mental health concerns by connecting them with the programs or resources that can provide the best care. Highly trained Family Consultants work within each home to provide intensive care coordination services. Ultimately, Boys Town empowers parents and caregivers to address family challenges such as trauma, mental health concerns, and academic failure, ensuring that children are safe, able to stay with their families, and receive the services they need.

The Care Coordination Services program primarily serves families who are low-income and who often face additional challenges including trauma, limited support systems, low father engagement, and untreated developmental and mental health concerns. Historically, over 70% of families served through Care Coordination Services in Palm Beach County are single mother led households. We recognize that these households face additional barriers to stability and success, and we partner with families to help them thrive.

The Boys Town Care Coordination Services model is supported by practice-based and research-based evidence. To ensure effective and quality program delivery, Care Coordination Services are thoroughly evaluated. Family progress is measured by conducting assessments at the beginning and end of services, and successful linkages to community resources are noted throughout service delivery. The vast majority of families served through this program report meaningful, positive outcomes. Historically, 85% of families remain intact, meaning safety issues were resolved and the children were not removed from the family home. In addition, 80% of families successfully complete the program, meeting all, or most, of their family service plan goals.

Sources For The Project (Committed & Potential): Kingdom Charitable Trust – $32,500 – Committed (November 2023 – October 2024). We receive additional funding from South East Florida Behavioral Health Network, Financially Assisted Agencies Palm Beach County, and Medicaid.
