Project Title: Jacob’s Heart Teen Group – a safe inclusive space for underserved teens to develop crucial life skills
Project Description:

We exist to improve the quality of life for children with cancer by supporting their families in the challenges they face. The majority of Jacob’s Heart children (80%) come from low-income homes with high levels of unmet, basic needs. 100% of our Teen Group participants are Latino and encounter systemic economic, cultural, and language barriers that further disenfranchise and marginalize young adults in need of mentorship and social support.

During the adolescent years, young people undergo significant changes and require increased emotional and social support to navigate this transformative period. Jacob’s Heart respectfully requests a $10,000 grant for our Teen Group – a special model that honors the resilience of socio-economically disadvantaged teens. Curriculum topics promote healthy lifestyles, habits, and attitudes – supported by therapy, coaching, and counseling available on site.

This much-needed Teen program has doubled in size over the last year. With your support, we will scale the size and impact of the group to include more teens by applying 25% of the grant toward our internal resources (staff time) and 75% toward expanding the budget for gatherings and activities.

Funds will support personal growth for 26 teens through facilitated activities that improve essential life skills, including leadership, self-confidence, communication, team building, and trust.

-Ongoing shared experiences and acceptance though mentorship and emotionally supportive peer relationships
-Improved self-confidence uplifted by peer’s welcoming and affirmation
-Improved life preparedness from 1:1 support and resources provided during key milestones (e.g. preparation and transportation to their first job interview)

Project Duration: The Teen Group meets monthly and our behavioral health team offers deep psychosocial support, crisis counseling, and connection seven days a week, year round, free of charge. If awarded, grant funds will be fully utilized in the year following the award.
Geographical Area Served: Jacob’s Heart serves all eligible families of children with cancer and families bereaved by the loss of a child living in South Santa Clara, San Benito, Santa Cruz and Monterey counties in California with a comprehensive level of support on referral from local pediatricians, Kaiser Permanente, Lucile Packard Children’s Hospital at Stanford and Benioff Children’s Hospital at University of California San Francisco.
