Project Title: Transitional Housing for At-Risk Youth
Project Description:

Since 1998, New Creation Home Ministries has provided housing and parental support services to homeless at-risk pregnant and parenting mothers ages 13-22. Our mission is to foster long-term independence through quality programs designed to encourage academic, vocational, parental, and social growth and success. NCHM houses a maximum of seven mothers and their children at any given time between our two homes; Lighthouse, where mothers stay for the first phase and have access to 24-hour care and services, and New Creation Home, where they move after one year to begin planning for and transitioning to independence. NCHM’s 2-year residential program implements a case management model that addresses trauma and adverse childhood experiences (ACEs) by assessing and designing individualized 90-day case plans. Mothers set both long-term and short-term goals in personal, academic, and financial development and then meet weekly with their case manager to track progress. These goals serve as re-education tools, measurements for success, and work as motivation. The young mothers we serve have experienced many Adverse Childhood Experiences (ACEs), impacting their ability to live sustainable lives. All have experienced abuse in relationships, either with their family of origin or the fathers of their children. They have come from chaotic, unsafe homes, where the turmoil of their environment hinders them from making the best life choices for themselves and their children. Many lack the skills to not live in poverty. The unfortunate impact shows in poor performance in school, poor decision-making, risky behaviors, and emotional and psychological instability, which ultimately leads to their housing instability.
Our primary objective is to see our residents reach self-sufficiency and independence. We estimate that of the women served through our Transitional Housing, at least 70% will successfully achieve self-sufficiency and increased livelihood, whereas without it 60% would likely experience chronic homelessness and/or continued abuse.
