Project Title: Inspire – Nutrition Education and Healthy Food Access
Project Description:

Children’s Health Center’s (CHC) mission is "Improving health, healing people.” We provide care to children with special needs, acute illnesses, and chronic disabilities/conditions. CHC is the only one of its kind in northern Arizona serving 50,000 square miles. Our Inspire program is an intensive health behavior and lifestyle treatment program specializing in the treatment of obesity, and conditions commonly associated with obesity, including Type 2 Diabetes, Fatty Liver Disease, Hypertension, Hyperlipidemia, Obstructive Sleep Apnea, Orthopedic conditions, and more. The program provides individualized, comprehensive care with a nurse practitioner, registered dietician, exercise physiologist, and social worker to foster healthy practices. Cooking classes are a vital educational tool to support nutrition education in children. We also invite the parents to participate as we believe health is a family affair, and parental support is crucial for the child or young adult to succeed and achieve their full potential. Currently we have a recurring grant with Sprouts Healthy Communities Foundation to support this effort and have received it six years in a row due to its success. We would like to expand our efforts to support more children. Through this request, we can offer these healthy life skills through 150 additional classes. This would cover the class food and a gift certificate at each visit to a local grocery store to purchase fruits and vegetables as prescribed by the dietician. In addition, each family would receive a cooking tool at their first class so they can replicate the recipes at home.
Food for 150 cooking classes at $25/first class + 50 follow-up classes = $3,750
Cooking tool for each first class at $40/child (100 classes) = $4,000
$10 gift certificates to local grocery store to purchase prescribed fruits and vegetables (150 cooking classes) = $1,500

Total Project Budget: $57,000 (this accounts for the Dieticians hourly charge)
