Project Title: Family Disability Navigator – Setting up Youth with Disabilities and their families for success in school
Project Description:
Our Family Disability Navigator program supports youth ages 3–22 and their families in accessing the services they need to succeed at home, in school, and in their communities. This program is built on individualized pathways, recognizing that each child and family has unique needs. Support may include coordinating a disability diagnosis, navigating 504 Plans or IEPs, and connecting families to essential resources such as food assistance, therapies, and community-based services.
Launched in 2022 in response to a community need we identified along with other funders and community partners, this program addresses the challenges children with disabilities face alongside their families. Navigating these systems can be overwhelming, particularly for families who are low-income, non-native English speakers, individuals with disabilities themselves, or those facing other barriers. At the same time, school-based resources are often limited, leaving families without the guidance they need.
Our team of three dedicated Navigators works one-on-one with children and their families to ensure they are supported every step of the way. We provide personalized guidance, advocacy, and coordination so that families are not navigating these complex systems alone.
Key services include information and referral for formal disability diagnosis; planning, advocacy, and navigation of IEPs and 504 Plans; connection to community resources; wellness assessments and referrals for children; parent education and support groups; case management; and skills training.
Through this work, we help families build stability, increase access to critical services, and create stronger outcomes for children with disabilities.
