Project Title: Enhancing Adolescent Wellness: Comprehensive Clinical and Mental Health Support at the Dr. Robert Appleby School-Based Health Centers (SBHC)

The Human Services Council (HSC) requests $10,000 to support the Dr. Robert Appleby School-Based Health Centers (SBHC), which provide critical medical and mental health services directly within Norwalk\u2019s middle and high schools. By offering "one-stop" healthcare, we eliminate traditional barriers to access\u2014such as lack of transportation, insurance gaps, or parental work schedules\u2014ensuring that students receive high-quality care without missing significant instructional time.<\/p>\n
The SBHCs are staffed by multidisciplinary teams, including Mid-level Practitioners and Licensed Clinical Social Workers. Funding from the Hermanowski Foundation will specifically support the delivery of integrated care:<\/p>\n
Mental Health Services: Individual and group counseling to address rising rates of anxiety, depression, and trauma among adolescents.<\/p>\n
Clinical Medical Care: Physical exams, immunizations, and acute care for illnesses or injuries.<\/p>\n
Health Education: Proactive wellness coaching to encourage lifelong healthy habits.<\/p>\n
Our centers serve as a vital safety net, particularly for students who are underinsured or uninsured. By addressing health issues early and in a familiar environment, we reduce ER visits and improve school attendance. This grant will help cover the gap between insurance reimbursements and the actual cost of providing intensive, trauma-informed care to Norwalk\u2019s most vulnerable youth, ensuring that every student has the physical and emotional foundation needed to succeed academically.<\/p>\n
