We are seeking funding support from the Hermanowski Family Foundation to facilitate a free community workshop focused on educating families about health and wellness.

Purpose of the Program:
The proposed program aims to deliver valuable educational sessions, resources, and services to families in need, addressing important topics such as nutrition, exercise, self-care, and mental health. Through expert-led workshops and training classes, we aim to equip participants with the knowledge and tools necessary to make healthier choices and lead a more fulfilling lifestyle.

The utilization of Grant Funds is as follows:

1. Experts in the Field: A portion of the grant funds will be allocated to inviting qualified health professionals and educators to conduct informative sessions and workshops during the program. Their expertise will ensure high-quality content delivery and engaging interactions with attendees.
2. Program Coordinator: Hiring a skilled program coordinator will ensure the seamless planning and execution of the workshop. The coordinator will oversee logistics, coordinate with speakers and facilitators, and manage participant registration.
3. Venue Rental: Another portion of the grant will be used to secure a suitable venue for hosting the community workshop. The venue will be selected based on its accessibility to the target audience and capacity to accommodate the expected number of participants.
4. Giveaways for Families: A portion of the grant funds will be allocated towards providing giveaways and informational materials to families attending the workshop. These resources will serve as tangible reminders of the lessons learned and encourage continued engagement with healthy practices.
5. Follow-up Training Sessions: The remaining grant money will be invested in organizing training classes aimed at imparting practical skills and strategies related to health and wellness. These classes will offer continued support and hands-on learning opportunities for participants to adopt healthier habits and routines.

Expected Impact:
By supporting this initiative, the Hermanowski Family Foundation will directly contribute to the betterment of our community by promoting health literacy, fostering healthy behaviors, and empowering families to lead healthier lives. The program will serve as a valuable resource for families seeking guidance on improving their overall well-being and creating a supportive network within the community.

We are confident that with the foundation’s generous support, we can successfully execute this educational health and wellness program and make a meaningful difference in the lives of those we serve. Your partnership in this endeavor will greatly benefit the community and align with the foundation’s commitment to promoting health and wellness initiatives.

Thank you for considering our grant proposal. We look forward to the opportunity to discuss this further and explore how we can collaborate to create a positive impact on the community.

Total Project Budget: $5000

Other Funding
Sources For The Project (Committed & Potential): New York Life Insurance, New Jersey Citizen Action, Exodus- A Program of Urban Behavioral health Services

Project Duration: (Bi-Annual Event) Preparatory Phase- 4 Weeks, Implementation Phase 1 Day, Follow-up and Evaluation Phase 2 Weeks -Total 6 weeks
Geographical Area Served: Primarily Jersey City, NJ Local community centers, schools, and organizations from diverse backgrounds , targeting our homeless and economically underprivileged families
Age Group To Be Served: General population served ranges from Children to Seniors

Contact Information

Contact Prefix (Mr,Mrs etc.): Ms.
Contact First Name: Tanisha
Contact Last Name: Wright
Contact Title: Executive Director
Contact Phone: 201-898-0851
Contact Email: zioncaresjc@outlook.com

Hermanowski Family Foundation Initial Request Form Organization Name: Zion Cares Community Development Corporation Legal Name (if Different): Also Known As: Zion Cares CDC Mailing Address: 521 Bramhall Avenue City: JERSEY CITY State: NJ Postal Code: 07304 Main Phone: 2018980851 Main Fax: Organization Website: Https://www.zioncaresjc.org Employer ID Number: 86-1542721 Organization Tax Status: Nonprofit 501 c 3 […]

[3

post-4828","post","type-post","status-publish","format-standard","hentry","category-grants
["GET
