Project Title: Health Hub
\nProject Description:<\/p>\n
The CHMRC Health Hub serves as a comprehensive bridge to health equity for these underserved populations. In 2023, we laucnehd the Health Hub that operate on a calendar year from January to December. We collaborate with public health partners, local community-based organizations, and faith-based organizations to provide community members wellness programs, health workshops, and support groups on weekdays from 9 am to 6 pm. We aim to serve some 1,000 program participants including youth and adults from underserved immigrant and minority populations.
\nOur Health Hub programs are implemented by two highly skilled and dedicated Community Resilience Coordinators. Our coordinators serve the community, provide culturally and linguistically appropriate health information, and establish trust by engaging with families and identifying their barriers and health concerns. Weekly, the coordinators outreach to potential community partners and coordinate community events and workshops to promote health education and resilience-building skills. They provide vaccinations, health screenings, chronic disease management, and crucial support groups like parenting and babysitting classes. Workforce readiness training and mental health resources empower individuals to build a healthy future. Through collaborative partnerships, we equip them with knowledge, tools, and referral services to navigate local resources and connect with learning opportunities.<\/p>\n
Our impact extends beyond health education. We alleviate immediate burdens with food bags, clothing, and hygiene kits, allowing individuals to focus on their health and well-being. The Hub also fosters community connections: local organizations utilize our platform to connect with health and wellness resources, strengthening the safety net for all.<\/p>\n
The Health Hub removes barriers to health and wellness and equips individuals with the knowledge and tools to make informed choices for themselves and their families.<\/p>\n
