This is part one of the curriculum.<br \/>\n\u2022\tGuess speakers and field trips will be implemented into the program<br \/>\nTarget Populations<br \/>\nTeenagers, and At -Risk Youth ages 13-18 who live in Oakland and Alameda County.  <\/p>\n<p>Outcomes<br \/>\nDeliverables: Submit to funder, within 12 months of receiving final budget clearance, a comprehensive Planning and Implementation Guide detailing and mapping the project.<br \/>\nFunding will be used: Implementation and\/or training of staff on using validated assessment tools with model fidelity and high interrater reliability to effectively assess individual youth\u2019s unique risk factors and needs. To provide a comprehensive, collaborative services to ensure youth\u2019s success which could include:<br \/>\n\u2022\tA description on how the youth and family will directly be involved in developing the individualized education plan.<br \/>\n\u2022\tA description on how facility and community-based providers will implement a standard in the planning process.<br \/>\n\u2022\tA description of the process and timeframe that the facility and provider will use to develop and implement a humane and individualized plan that directly involves youth and family participation.<br \/>\n\u2022\tTo provide a plan and description that includes youth voice and family engagement in planning.<br \/>\nDelivery of transitional services to help reentering successfully integrate into their communities, which could include:<br \/>\n\u2022\tEducational, literacy, and vocational services and transitional job strategies<br \/>\n\u2022\tSubstance use disorder treatment and services<br \/>\n\u2022\tCoordinated services, including physical and mental healthcare<br \/>\n\u2022\tPermanent supportive housing<br \/>\n\u2022\tFamily services<br \/>\n\u2022\tProsocial activities<br \/>\n\u2022\tMentoring<br \/>\nYouth Training and Technical Assistance<br \/>\nThe training and technical assistance provider will provide support and guidance to Youth grantees and practitioners to improve community supervision practices, produce better outcomes for youth and communities, reduce recidivism, improve public safety, and enhance jurisdictions\u2019 access to the tools and training to adequately identify, as well as effectively serve, these youth\u2019s needs.<br \/>\nThe multidisciplinary team are experts in implementation science, community supervision, adolescent development, trauma-informed care, data analysis, and performance measurement will help the selected jurisdictions improve their community supervision programs, policies, and practices. Applicants will be required to address the following three areas:<br \/>\n\u2022\tProgram will consider youth\u2019s needs, as well as a youth\u2019s strengths and assets by utilizing a strengths-based approach that involves getting to know a youth\u2019s culture, qualities, characteristics, interests, skills, talents, abilities, and family connections.<br \/>\n     Youth Fellowship Program:<br \/>\nThe Youth provider will develop a fellowship program.
