Without these physicals, children cannot start school or play sports. Children are required by Florida state law to demonstrate proof of immunization and an annual examination, prior to enrollment in a Florida state public school.
Now more than every it is important to get children connected to health providers. To date we have not changed the fact that the second-highest number of children without health insurance live in Florida. Last year, Shepherd’s Hope ensured physicals for 340 children. This year we forecast serving up to 800 children of the uninsured families.
Shepherd’s Hope respectfully requests the Charles and Joan Hermanowski Family Foundation to join us in increasing the reach and impact of the Back-to-School program that has become a vehicle to bridge the coverage gap by providing free access to high quality healthcare to underserved and uninsured children while mitigating the tremendous financial implications to the community and other healthcare providers.

Total Project Budget: $2,977,473

Other Funding
Sources For The Project (Committed & Potential): Harper Family Foundation ($15,000), AL & Nancy Burnett Charitable Foundation ($20,000), The Bond Foundation ($ 15,000), Bank of America ($100,000), ATAP Companies ($50,000). *The contributions indicated are from the last calendar year.

Project Duration: May 1 – Sept 30
Geographical Area Served: Central Florida (Orange, Seminole, Osceola and Lake counties)
Age Group To Be Served: 4-15 and 16+

Contact Information

Contact Prefix (Mr,Mrs etc.): Mrs.
Contact First Name: Ayesha
Contact Last Name: Butt
Contact Title: Grants Manager
Contact Phone: 407-876-6699
Contact Email: ayesha.butt@shepherdshope.org

Hermanowski Family Foundation Initial Request Form Organization Name: Shepherd’s Hope Legal Name (if Different): Shepherd’s Hope Also Known As: SH Mailing Address: 455 9th Street City: Winter Garden State: Florida Postal Code: 34787 Main Phone: 4078766699 Main Fax: Organization Website: www.shepherdshope.org Employer ID Number: 59-3420727 Organization Tax Status: 501 c 3 Proposal Information Today’s Date: […]

[3

post-1306","post","type-post","status-publish","format-standard","hentry","category-grants
["GET
