Transplant Families emerged out of a pursuit to inspire and support the community that we too are a part of. We have provided support, education, and inspiration to families across the United States.

Each year Transplant Families hosts a virtual conference to connect families with providers to foster lasting relationships of learning, connecting, and growing through the stages of the pediatric transplant journey. Our goal every year is to bring current experts in the field that impact transplant families directly. Additionally, these conferences help raise money for the distribution and contents of our Welcome Bag Program, scholarships for families that need financial help, and training for our staff members.

Our Welcome Bag Program consists of families a bag full of items that we found helpful when our children were in the hospital; A tote/backpack, water bottle, lip balm, phone charger, hand sanitizer, pen & paper, cloth mask, Howl the Owl book, and window clings. We are always looking to add items as they become available by donations. The bags are delivered by members of the clinical transplant team when it is appropriate for the family. We expect these bags to be delivered to at least 50 of the highest volume pediatric transplant centers, and eventually, expand to all 107.

We are asking for $10,000 in grant funding to help Transplant Families expand our reach to new families and pediatric transplant centers. This funding will be used to help fund our virtual conference, our Welcome Bag Program, financial scholarships for families, and Diversity and Equity staff training.

Total Project Budget: $10,000

Other Funding
Sources For The Project (Committed & Potential):

Project Duration: 2023 Virtual Conference (April 2023)
Geographical Area Served: Throughout the United States
Age Group To Be Served: Under 18

Contact Information

Contact Prefix (Mr,Mrs etc.):
Contact First Name: Melissa
Contact Last Name: McQuees
Contact Title: Board President
Contact Phone: 623-217-5875
Contact Email: melissa@transplantfamilies.org

Hermanowski Family Foundation Initial Request Form Organization Name: Transplant Families Legal Name (if Different): Also Known As: Mailing Address: 24654 N Lake Pleasant Pkwy #103- 187 City: Peoria State: AZ Postal Code: 85383 Main Phone: 623-777-9893 Main Fax: Organization Website: https://www.transplantfamilies.org/ Employer ID Number: 82-2064952 Organization Tax Status: 501(c)3 Proposal Information Today’s Date: 2/18/2022 Requested […]

[3

post-1068","post","type-post","status-publish","format-standard","hentry","category-grants
["GET
